Because a parent or legal guardian will not be present at check-in, we need this consent completed before your rider arrives. This verifies identity and gives us the legal authorization required for minors to participate in electric dirt bike activities at SoTah Adventures.

Supervising Adult / Chaperone Information

I, the parent/legal guardian of the above minor, authorize the supervising adult named above to act inloco parentis (in the place of a parent) for the purpose of participating in activities operated by SoTah Adventures, LLC.

This includes authority to:
• Supervise my child during all SoTah Adventures activities.
• Sign and execute the SoTah Adventures liability waiver and any required documents on my behalf.
• Make routine and emergency decisions regarding my child’s safety.
• Approve my child riding in a SoTah Adventures–operated or approved UTV/ side-by-side (SXS) vehicle for emergencies, transport, or extraction.
